We are Back HOME!
Our new state-of-the-art facility houses our Information and Referral Specialists’ who provide community members with the appropriate resources and referrals to increase their housing stability. This office will also offer access to free fax machine, a notary, fresh produce and nutritious food through our Fresh Market in partnership with Lake View Pantry and our health services through our Health Clinic in partnership with Howard Brown Health. It also houses all of our direct service program staff including DCFS, Housing Advocacy and Case Management, Palante Case Management, Rapid Re-Housing, and our Nutrition Center.
Our Solid Ground Supportive Housing Program
provides male-identified youth, ages 16-21 who are homeless with housing for up to two years. Staff in this program further support residents by offering the skills they need to maintain stable housing and employment, rebuild their self-esteem, address barriers to stability and prepare them for continuing education.
Our Casa Corazon Program
works with homeless and high-risk youth and is based in Humboldt Park, Logan Square and Back of the Yards communities. The program provides a daytime drop-in center where youth have access to food and support as they work towards stable housing, employment, and educational achievement. We also offer 45 emergency beds for youth ages 18-24 experiencing homelessness at two shelters which open daily at 9:00 pm and provide dinner, a bed to sleep in, breakfast and CTA transportation passes to clients.
